Your liver is a large football-shaped organ in your upper abdomen. During a liver transplant, surgeons disconnect your liver's blood supply, including the portal vein, and the biliary system, including the bile duct. The liver is removed and replaced with a donor organ. The blood supply and biliary system are connected to the new liver.
